I authorize Hector Blanco (and the team at Vale Insurance, LLC) to perform the following functions:

1. Create and/or access my account with the Health Insurance Marketplace
2. Apply for financial assistance in order to purchase health insurance
3. Enroll in a plan that best suits my/our needs
4. Perform follow-up activities during the plan year 2025
